FOSTER'S AGREEMENT AND RELEASE

**Please read carefully and initial each statement!**

Happy Cats Haven retains all rights and ownership of any animal in the Foster Program. Happy Cats Haven has the right to recall any fostered animal to the shelter at any time.*

Foster volunteers must attend a Happy Cats Haven orientation prior to fostering.*

I will maintain adequate food and water for my foster cat(s) as well as check them regularly for any signs of illness.*

Happy Cats Haven makes all decisions regarding animals to be placed in foster. I will not incur any expenses related to the cat, including veterinary care, without the prior express consent of Happy Cats Haven. If I incur expenses without prior consent, I understand that I am personally responsible for those expenses.*

I agree to arrange my family’s schedule so that the pet is seldom left alone for more than eight hours at a time. I agree to play with, pet and give attention to my foster pet when I am home.*

I will closely supervise this cat when around children (especially anyone he/she is not familiar with). If my foster cat happens to escape his/her confinements while in my care, I agree to inform Happy Cats Haven immediately and try to locate the foster pet.*

Foster animals must be returned to Happy Cats Haven as soon as they are well or upon request by HCH. I agree that if at any point I decide that I cannot keep the cat for any reason at any point during the foster period, I will return him/her to Happy Cats Haven.*

Only authorized foster volunteers may care for foster animals; foster animals must not be left under anyone else’s care without authorization from HCH.*

Happy Cats Haven makes no warranties with regard to the health or temperament of the cat and disclaims any liability for the cat's medical condition, temperament, sickness or disease.*

Happy Cats Haven is required by the State of Colorado to visit foster homes yearly. We will arrange that visit with a prior notice of at least 3 days.*

Prior notice is not required for the repossession of an animal when a foster volunteer fails to bring the animal back to the Happy Cats Haven as requested.*

I will relinquish the cat upon request. I also understand that, if for any reason, I am unable to continue fostering a particular cat, I will give Happy Cats Haven adequate time to find the cat another foster.*

I release and forever discharge Happy Cats Haven, its Board, volunteers, foster homes and representatives, and the veterinarian treating the cat at the request of Happy Cats Haven from any and all claims and actions, including, but not limited to property damage caused by the cat, personal injury to me, family members, third parties or other animals from biting, scratching, transmittal of disease, and/or any other matter attributable to said cat. Any cat has the potential to bite or cause injury. I further agree to indemnify and hold harmless Happy Cats Haven and its Board, volunteers, foster homes and representatives, joint and severally, from any and all actions, claims, and demands for, upon or by reason of any damage, loss or injury, which hereafter may be sustained by fostering a cat.*

I UNDERSTAND AND AGREE TO THE ABOVE TERMS. I UNDERSTAND THAT IF I KNOWINGLY FAIL TO COMPLY WITH THE ABOVE TERMS, OR IN ANY WAY PUT THE ANIMAL IN A SITUATION WHICH ENDANGERS HIS OR HER HEALTH, HAPPY CATS HAVEN MAY TAKE APPROPRIATE ACTION, INCLUDING LEGAL ACTION, TO TAKE THE PET FROM MY POSSESSION.*
